Job Description

We are seeking a dedicated and compassionate Community Health Worker to join our team and play a pivotal role in advocating for the health and well-being of community members through an integrated approach to care management and community outreach. As a bridge between healthcare providers and the community, the ideal candidate will empower individuals to adopt healthier lifestyles and access the resources they need. Your responsibilities will include conducting outreach and education programs, providing support to patients navigating the healthcare system, and facilitating access to various health services, including mental health and social services. You will work closely with local organizations and government agencies to develop community health initiatives aimed at reducing health disparities and improving overall health outcomes. The Community Health Worker will be instrumental in collecting data and feedback from the community to inform health programs, ensuring they meet the specific needs of residents. Job Responsibilities
  • Establish trusting relationships with patients and their families while providing general support and encouragement.

  • Help patients set personal health related goals and attend appointments.

  • Work closely with medical providers to help ensure that patients have comprehensive and coordinated care plans.

  • Facilitate communication between community members and health care organizations.

  • Build a trusting relationship with the community you represent.

  • Help people understand their health or social conditions.

  • Making home visits to high-risk patients, pregnant or nursing mothers and the elderly.

  • Hold groups and individual meetings with people in the community.

  • Deliver health care information in a way that is easy to understand.

  • Help patients navigate healthcare and social service systems.

  • Reduce social isolation among patients.

  • Determine eligibility and enrol individuals in health insurance plans.

  • Ensure cultural competence among healthcare providers serving vulnerable populations.

  • Educate healthcare providers and stakeholders about community health needs.

  • Provide culturally appropriate health education on topics related to chronic disease prevention, physical activity and nutrition.

  • Advocate for underserved individuals or communities to receive services and resources to address health needs.

  • Collect data and relay information to stakeholders to inform current and future programs and policies.

  • Provide informal counselling, health screenings and referrals

  • Build community capacity to address health issues

Skills, Requirements and Qualifications
  • Strong interpersonal and communication skills, with the ability to interact effectively with patients and healthcare team members.

  • Ability to multitask, prioritize responsibilities and work efficiently.

  • Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare practice.

  • Ability to work as part of a team.

  • Willingness to undergo additional training and development as required.

  • Ability to work with people in a group and individual basis.

  • Ability and willingness to interact with community elders and clients.

  • Willingness to work extensively in the field under minimum supervision.

  • A good communicator with excellent public speaking skills.

  • The ability to speak in the local dialect is an added advantage.

  • Passionate about working with poor rural households in remote areas is a great plus.
